Before your first visit we need some contact and health information.

What to Bring to Your First Appointment
- Your referral letter
- A completed "New Patient" Form (fill in the New Patient form online now or you can Click Here to download)
- All relevant scans and xrays (especially sinus CT scans)
- All test results (hearing tests, skin prick tests, etc)
- Medicare card, DVA card, Pension Card
- Private Hospital Insurance Information
To Cancel an Appointment
Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
Waiting Times
We recognise that your time is valuable, and we make every effort to run on time. Occasionally, emergencies or complex patients require a little more time, and these may cause scheduling delays beyond our control. We apologise if we keep you waiting.
Office Location
Parking
Please enter from Norbrik Drive. There are over 600 parking spaces on-site (per-hour fee). On-street parking is free. You may also park for free in the nearby "Circa" Shopping Centre (5-10 min walk).
Suite G6 Location
We are located on the GROUND floor at the END of the long corridor. Please arrive 10-15 minutes early for your Appointment for registration and to fill in a New Patient form.
